Session Description:

In this session, Agrace's DEI team will discuss what encompasses "providing inclusive care" in a hospice and palliative care environment. We will discuss implicit bias, perception, and how to counteract these elements when they surface in our work.

    Meet Our Presenter:

Troy Polnitz, DEI Manager, Agrace Hospice

Troy Polnitz (he, him, his) serves as the Diversity, Equity and Inclusion Manager at Agrace Hospice and Palliative care. Alongside his DEI Coordinator Alejandra Najeras, Troy has led DEI efforts that center around the A.C.E acronym, Activism, Community and Education. Troy’s team has been dedicated to weaving these three elements throughout the organization’s goals, as well as creating equitable approaches to processes regarding staff, patient and families. Troy’s team focuses on providing inclusive access for all and ensure that patients and families from marginalized groups are receiving the end-of-life care that they deserve.
